The Office of Vital Statistics offers certified birth certificates for persons born from 1930 to the present. We have two offices to serve you in Eustis, Clermont and Leesburg. Note: The Leesburg location provides Birth and Death Certificates every 3rd Tuesday from 9 a.m. - 12 p.m. Due to office closure these services will not be provided at this time in Leesburg.

To apply for a birth certificate:

You must be a parent listed on the certificate, a legal representative, a legal guardian (with court appointed papers), or the individual listed on the certificate.
You must be at least 18 years of age, and must have state issued identification.
Certificates are available for the State of Florida only. Applications for other states may be obtained from our office.
