Birth Certificates

Florida Department of Health in Lake County

The Office of Vital Statistics offers certified birth certificates for persons born from 1930 to the present. We have two offices to serve you in Eustis, Clermont and Leesburg.  Note:  The  Leesburg location provides Birth and Death Certificates every 3rd Tuesday from 9 a.m. - 12 p.m.

Birth Certificates can also be purchased at Lake County Tax Collector Office.

To apply for a birth certificate:

  • Fill out our form Application for a birth certificate (PDF 141KB)
  • You must be a parent listed on the certificate, a legal representative, a legal guardian (with court appointed papers), or the individual listed on the certificate.

  • You must be at least 18 years of age, and must have state issued identification.

Certificates are available for the State of Florida only. Applications for other states may be obtained from our office.

Birth Certificate $15

Each additional Birth Certificate $10

Vinyl Protector 8 1/2" x 11" $5 each

The Florida Department of Health in Lake County does not accept credit cards for online orders; however, for your convenience, you can process online requests through an independent company that we have partnered with to provide you this service; VitalChek Network, Inc.
