Eustis, FLThe Florida Department of Health in Lake County is inviting the public to help support a culture of health in the community.  Opportunities to participate include upcoming community health improvement plan (CHIP) webinars, team events, and focus group meetings.

10:00 AM – 11:00 AM (SUBSTANCE ABUSE focus group)

11:00 AM – 12:00 PM (ACCESS to FOOD & NUTRITION focus group)

2:00 PM – 3:00 PM (ACCESS to PRIMARY CARE focus group)

3:00 PM – 4:00 PM (DIABETES focus group)
