DOH-LAKE COVID-19 IMMUNIZATION SITES UPDATE FOR 03/26/21

Eustis, FL. – The Florida Department of Health in Lake County (DOH-Lake) COVID-19 Immunization sites revised schedule for the week of March 29, 2021 through April 2nd, 2021:

St. Patrick Catholic Church Site located at 6803 Old Hwy 441, Mt. Dora. Tuesday – 9:00 a.m. to 1:00 p.m.

Wednesday - 9:00 a.m. to 6:00 p.m. (extended hours) Thursday - 9:00 a.m. to 1:00 p.m.

Open for 1st and 2nd doses on a first come, first served basis.

As a reminder, this will be the last week of operation for the Amazon Distribution Site located at 7453 Republic Drive, Groveland.  The hours of operations will be as follow:

Monday through Friday from 8:00 a.m. to 1:00 p.m. (Wednesday will have extended hours till 7PM)

Open for 1st and 2nd doses on a first come, first served basis.

Schedules are based on vaccine availability.  Dates and times are subject to change.

The Lake Square Mall immunization site is available by appointment only. Residents may preregister for an appointment online at myvaccine.fl.gov or by phone at 866-201-6909 (or 833- 476-1031 for TTY users).

During this phase, we’ll continue to vaccinate the following populations: Persons 40 years of age and older; Health Care personnel with direct patient contact; Long-term care facility residents and staff; Persons under 40 deemed medically vulnerable by a physician (RequiredForm)
