Clinical Services

Florida Department of Health in Lake County

We provide a variety of quality health care and support services.


All new and existing clients that need eligibility or re-certification will need to go through our new Eligibility process. You will need to have eligibility determined for services PRIOR to making your clinic appointment. If applying for children, a social security card for yourself and each child is required.

  • Dental Services

Lake Children's Dentistry has two locations:

Fruitland Park and Clermont Health Center

Serving children ages 6 months to 20 years of age

  • Immunization
The Florida Department of Health in Lake County protects the health and safety of the community by providing child, adult and travel immunization services. Our immunization staff are subject matter experts and place an emphasis on educating the public on the importance of vaccines.
  • Medical Records

What You Need to Bring:

  • Your photo ID
  • For records for a minor, bring guardianship paperwork (if guardian is other than parent)

Services include:  Family Planning, Maternity, STI Testing and Treatment, Pediatrics, Immunizations, Work and School Physicals, Quick Care, Travel Immunizations, HIV, Epidemiology/Disease Control, Tuberculosis, Refugee Health, Breast and Cervical Cancer Early Detection, Sharps Container Disposal Program, and ADAP.

Services include:  Dental

  • School Health
In cooperation with the Department of Education, the School Health Services Program provides the services mandated in Florida statute sections 381.0056, 381.0057, and 402.3026. School health services are intended to minimize health barriers to learning for public school students in pre-kindergarten through 12th grade.
  • WIC
WIC is a federally funded nutrition program for Women, Infants, and Children. WIC provides the following at no cost: healthy foods, nutrition education and counseling, breastfeeding support, and referrals for health care and community services.
