A community health improvement plan is a long-term, systematic effort to address public health concerns in a community. The plan is based on the results of health assessment activities, and is part of a community health improvement process.

The 2017-2020 Lake County CHIP uses the Mobilizing for Action through Planning and Partnerships, or MAPP, model.

Public health belongs to the community. In order to create the conditions in which people can be healthy, communities must collectively address social, economic, environmental, and biological factors. Improving the public’s health requires the expertise of all those who live and work in the community, and all entities make important contributions to the local public health system.  Learn more about the local public health system.

MAPP - Your community Roadmap to Health

The Florida Department of Health in Lake County facilitated a community-driven strategic planning process of improving health by organizing a series of three workshops in September through November of 2016. A consensus workshop is a structured facilitation method to help groups think, talk and work together.

Using the musical theme, A "Symphony of Health", the workshops moved the Lake County community from health planning into action and implementation with consensus. The process results in a new community health improvement action plan for the next three years.

Local Public Health Assessment

The Florida Department of Health in Lake County facilitated a county-wide Local Public Health System Assessment (LPHSA). A local public health system assessment is a process which evaluates and documents how well the local public health system fulfills the ten essential services of public health. The assessment was completed in a series of five weekly workshops which ran through April and May of 2016. Participants representing organizations whose combined activities contribute to the health and wellbeing of the community in Lake County reviewed and assessed performance based on a set of national standards.

Participants of the consensus workshops reviewed local community health assessments and had some courageous discussions about strategic directions moving forward.

Implementation Plans:  Strategic planning on how to address issues in a specific and measurable way.

Impact Reports:  Report about an agency, group, program, project, etc. that details outputs and outcomes in the community.

  • Phase 4: Identify Strategic Issues

Participants of the consensus workshops used a prioritization matrix tool to pare-down from a list of 34 health issues identified in the community health assessments and rank the top ten health issues by perceived health impact.

  • Phase 5: Formulate Goals and Strategies

Participants of the consensus workshops suggested goals and identified evidence-based strategies for each of the top five focus areas of the community health improvement plan (CHIP) for the next three years.

  • Phase 6: The Action Cycle
