DOH-LAKE COVID-19 VACCINE SITES UPDATE for 01/11/21 – “New Location”

Eustis, FL. –Starting Monday, January 11, 2021 the Florida Department of Health in Lake County (DOH-Lake) will have a new COVID-19 immunization site in the northern side of the county.  Our new location will be at the St. Patrick Catholic Church located at 6803 Old Hwy. 441 S. Mount Dora, FL.

COVID-19 immunizations sites schedule for the Week of January 11th

Monday Thru Friday

9:00 a.m. – 4:00 p.m.

Hours will vary based on availability and are subject to change

Monday Thru Thursday

11:00 a.m. – 5:00 p.m.

Hours will vary based on availability and are subject to change

Clermont Arts & Recreation Center  

3700 S. Hwy. 27,

 prior to 7 a.m.

St. Patrick Catholic Church

Appointment options will be coming soon from DOH-Lake as well as our community partners

 

  • There will be a limited amount of spaces in our lot per day.
  • First come, first served
  • Drive-thru
  • Residents and non-residents
  • Persons 65+ will need identification
  • Frontline workers with direct patient contact will need to present a copy of their medical license, employee badge or current paystub.
  • Face masks must be worn while waiting in vehicles.
  • No referral needed. No cost. Clients will be asked to complete a registration form prior to receiving the vaccine.
  • Inclement weather like lightning, heavy rain, or gusty winds can slow operations or shut down the site.
  • Site will continue to operate based on vaccine availability

 

Second doses clients will be contacted when their second dose comes in and they will be given a separate walk-in day or an appointment.

If you have been on a blood thinners for less than a year or have had recent heart related surgery, take medications for atrial fibrillation, currently or any cancer treatments, cancer within the last six months, or take any immunosuppression medication please bring documentation from the provider stating that you may take the vaccine.
