DOH-LAKE COVID-19 IMMUNIZATION SITES UPDATE FOR 04/30/21

Eustis, FL. – The Florida Department of Health in Lake County (DOH-Lake) COVID-19 Immunization sites schedule for the week of May 3rd, 2021 till further notice:

DOH-Lake Community Health Site located at 560 W. DeSoto Street, Clermont. Monday thru Friday from 8:00 a.m. to 12:00 p.m.

 

DOH-Lake Umatilla Site located at 249 E. Collins Street, Umatilla. Monday thru Friday from 8:00 a.m. to 12:00 p.m.

  • Sites will be offering 1st & 2nd Pfizer vaccine doses
  • No appointments
  • Schedules are based on vaccine availability
  • Dates and times are subject to

 

The Lake Square Mall immunization site is available by appointment. Residents may preregister for an appointment online at myvaccine.fl.gov or by phone at 866-201-6909 (or 833-476-1031 for TTY users). As a reminder, the last day to receive your first dose at this site will be Tuesday, May 4th.

 

All Florida residents are now eligible to receive any COVID-19 vaccine as prescribed by the Food and Drug Administration. The Pfizer vaccine is authorized for persons 16 and up (all minors must be accompanied by a parent/legal guardian). The Moderna and Janssen (Johnson and Johnson) vaccines are authorized for persons age 18 and up.
