All new and existing clients that need eligibility or re-certification will need to go through our new Eligibility process. You will need to have eligibility determined for services PRIOR to making your clinic appointment. If applying for children, a social security card for yourself and each child is required.

List of Programs Eligible

Family Planning & Family Planning Waiver

Determine eligibility for women of child-bearing age who are interested in contraceptive methods (birth control). Clients may be eligible for Florida Medicaid Family Planning Waiver coverage after loosing full Medicaid for childbearing women ages 14 - 55. The waiver program provides limited family planning services for up to 24 months. We offer this service to all residents of Lake and surrounding counties. (Family planning Waiver does not cover Pregnancy Test)

  • Depo-Provera Injection

  • Birth Control Pills

  • IUD

  • Diaphram

  • Emergency contraceptive

 

Specialty Care HIV/AIDS

HIV/AIDS Program provides specialty and primary medical care for eligible HIV/AIDS patients. Eligibility is determined yearly.

 

Documents You Will Need to Bring

1. IDENTIFICATION: (1 of the following)

  • Florida Drivers License

  • Florida State ID

  • Resident Alien Card

  • Passport ID from county of Origin

2. VERIFICATION OF RESIDENCY: (1 of the following)

  • Florida Drivers License or Florida ID with Lake County address

  • Voter's Registration Card

3. INCOME VERIFICATION:

  • Pay check stubs or letter from employer on company letterhead (most recent 4 stubs if paid weekly and 2 if paid bi-weekly)

  • Unemployment Statement/Letter

  • Notarized letter(s) of contribution from anyone assisting you financially

  • Self employment Log Book / Income Tax Return

  • Child Support - copy of award letter or receipt of the amount

  • Social Security/SSD (except for SSI)

  • Bank statement (only for Social Security clients)

Failure to provide required information may delay the eligibility process.
