The Office of Vital Statistics provides certified birth certificates for individuals born from 1930 to the present.
Eligibility requirements include:
- You must be a parent listed on the certificate, a legal representative, a legal guardian (with court-appointed papers), or the individual named on the certificate.
- You must be at least 18 years of age.
- You must provide a state-issued identification.
Certificates are only available for births in the state of Florida. Applications for other states cannot be processed in our office.

Bring your completed application, state-issued identification, and payment. The Florida Department of Health in Lake County has three offices to serve you.
Processing the request in-person usually takes approximately 15 minutes.

Processing Time
Processing the request by mail is dependent on U.S.P.S. delivery. We process requests received daily, during regular business hours.

Processing Time
Processing time is based on the mail option you select. Certificates can be mailed through U.S.P.S. and usually take 3-5 days, or UPS delivery is available for an additional fee.
