Order death certificates online through VitalChek®, the only vendor authorized by the Florida Department of Health.
Death certificates and can be purchased at any Florida Department of Health’s (DOH) vital statistics office throughout the state. DOH only issues certificates for deaths that occured in Florida. If the death occured outside of Florida, contact the state in which the death occurred.
Family members or a legal representative(s) can request copies of a death certificate. Copies of death certificates listing the cause of death requires proper authorization.

Processing Time
Processing the request by mail is dependent on U.S.P.S. delivery. We process requests received daily, during regular business hours.

Copies of a death certificate can be processed online. DOH partners with VitalChek to service online requests. An additional fee is charged by VitalChek for using this service, and all major credit cards are accepted.

Processing Time:
Processing time is based on the mail option you select. Certificates can be mailed through U.S.P.S. and usually take 3-5 days, or UPS delivery is available for an additional fee.
