DOH-Lake Vital Statistic Office to Reopen July 8th, 2024

Eustis, Fla. –The Florida Department of Health in Lake County (DOH-Lake) Vital Statistic Office located at 16140 US Highway 441, Eustis, FL, will reopen on Monday, July 8^th, 2024.

The office will resume services with regular scheduled hours of Monday through Friday 8 a.m. to 4:30 p.m.

As a reminder, we have two additional office locations with the same hours of operations:

• DOH-Lake Leesburg

2113 Griffin Road, Leesburg, FL

• DOH-Lake Clermont

875 Oakley Seaver Drive, Clermont, FL
