Back to School Immunization Clinics

THE FLORIDA DEPARTMENT OF HEALTH IN LAKE COUNTY REMINDS PARENTS OF THEIR IMMUNIZATION SERVICES 

Lake County – To help parents and guardians prepare students to go back to school, the Florida Department of Health in Lake County (DOH-Lake) wants to remind parents of their immunizations services that are offered Monday – Friday at the following DOH-Lake Health Centers: 


Clermont Health Center                               
Thursday & Friday by appointment only

There is no charge for immunization for children, birth to 18 years of age provided they are still enrolled in elementary, middle or high school.  Parents or guardian must bring a copy of the child’s immunization record. In addition, we offer back to school physical examinations by appointments only.
