ST. STEPHEN AME CHURCH “BACK TO SCHOOL HEALTH FAIR” 

 
 
Lake County – In observance of National Immunization Awareness Month, St. Stephen AME Church will be hosting a “Back to School Health Fair” scheduled at the following location:
 
 
Volunteers will be providing Immunizations for school age children provided they are still enrolled in elementary, middle or high school.  Parents or guardian must bring a copy of the child’s immunization record. 
 
Volunteers will also be providing, vision, hearing and BMI screenings for students going back to school.  This event will also include an educational program, free school supplies, and the opportunity to visit informational booth.  No registration is required and there is no cost for the event. (parents must be present)
