Lake County – The Florida Department of Health in Lake County (DOH-Lake) is happy to announce that they will be offering vital statistics services at other DOH-Lake locations.  You will need to bring with you a valid state issued ID and cash or credit card. 
 
These satellites offices will be open every 3^rd Tuesday of the month at the following locations:
 
Tuesday, February 16 from 9 am to 12:00 pm
