EUSTIS, FL – The Florida Department of Health in Lake County (DOH-Lake) is happy to announce that they will be offering vital statistics services at other DOH-Lake locations.  You will need to bring with you a valid state issued ID and cash or credit card. 
 
These satellites offices will be open at the following locations:
 
Tuesday, July 19 from 9:00 am to 12:00 pm
Leesburg Health Center
2113 Griffin Road, Leesburg FL 34748
 
Thursday, July 28 from 1:00 pm to 7:00 pm
Umatilla Health Center
249 E. Collins Street, Umatilla 32726
 
The fees are:   Birth Certificate           $15.00 ea.
                        Death Certificate         $12.00 ea.
                        Vinyl Protector            $  5.00 ea.

We will continue to provide same day services from 8am to 4:30 pm Monday through Friday at our following locations:
