Community Health Improvement Planning Consensus Workshop Series  

Eustis, FL The Florida Department of Health in Lake County is inviting the public to participate on the county-wide Community Health Improvement Planning Consensus Workshop series at the Lake County Extension Services Building Auditorium, 1951 Woodlea Road, Tavares, FL  32778, from 9:00 a.m.-12:00 p.m.  Attendance at all three sessions is recommended.

Each meeting will focus on the following health planning areas with the musical theme, A Symphony of Health:
 

  • September 21, 2016, Finding the Chords, Common Themes and Visions
  • October 6, 2016, Creating the Score, Strategic Issues and Directions
  • October 20, 2016, Playing the Music, Focused Implementation
 
The consensus workshops are next steps for the community-driven strategic planning process of improving health using the Mobilizing for Action through Planning and Partnerships, or MAPP, model.  The process will result in a community health improvement plan (CHIP) for the next three years.
