FLORIDA DEPARTMENT OF HEALTH IN LAKE COUNTY ADMINISTRATION OFFICE TEMPORARILY CLOSED 

Eustis, FL– The Florida Department of Health in Lake County Administration Office, located at 16140 US Hwy 441, Eustis, FL, will be closed from Wednesday March 22, 2017 through Friday, April 7, 2017 while the office is renovated.

The Vital Statistics office will be temporary moved to our Environmental Health Office located at 315 West Main Street, Suite 523, Tavares FL.  If you have any questions or need to reach the administration staff during this temporary closure, please contact us at (352) 589-6424.   

The office will reopen Monday, April 10, 2017 with regular schedule hours.
