Eustis, FL– The Florida Department of Health in Lake County has rescheduled the November 29th, 2018 Community Health Improvement Planning (CHIP) fourth quarter meeting to Tuesday, December 11th. The format will also be updated to a face-to-face event.
The Florida Department of Health invites you to join the Lake County, Florida community on Tuesday, December 11th, 2018 at 1:00 PM. The meeting will be located at the Lake County Extension Services Building, Training Room B, 1951 Woodlea Road, Tavares, FL 32778.
