Eligibility

All new and existing clients that need eligibility or re-certification will need to go through our new Eligibility process. You will need to have eligibility determined for services PRIOR to making your clinic appointment. If applying for children, a social security card for yourself and each child is required.

  • Immunization
The Florida Department of Health in Lake County protects the health and safety of the community by providing child, adult and travel immunization services. Our immunization staff are subject matter experts and place an emphasis on educating the public on the importance of vaccines.
  • Medical Records

What You Need to Bring:

  • Your photo ID
  • For records for a minor, bring guardianship paperwork (if guardian is other than parent)

Clermont Health Center

Forms and Documentation: Monday - Friday 8 a.m. – 5 p.m.

Core Services: Mondays, Tuesdays (only the 1st Tuesday of each month) and Wednesdays: 8 a.m. - 5 p.m.

**Vital Statistics provided 8 a.m. – 4:30 p.m. (Closed 12pm - 1pm for Lunch)

Leesburg Health Center

2113 Griffin Road, Leesburg, FL 34788

Core Services: Tuesdays (except the 1st Tuesday of each month) and Thursdays: 8 a.m. - 5 p.m.

Core Services include Family Planning, Immunizations, Work and School Physicals, Quick Care, Travel Immunizations, Refugee Health, Breast and Cervical Cancer Early Detection, Sharps Container Disposal Program, Epidemiology/Disease Control, STI Testing and Treatment, Infectious disease care for Tuberculosis, PrEP, and nPep medication therapy for HIV prevention, HIV/AIDS treatment, and care. RWHAP-Ryan White HIV/AIDS Program Part A and ADAP support for eligible patients.

 

  • School Health
In cooperation with the Department of Education, the School Health Services Program provides the services mandated in Florida statute sections 381.0056, 381.0057, and 402.3026. School health services are intended to minimize health barriers to learning for public school students in pre-kindergarten through 12th grade.
  • WIC
WIC is a federally funded nutrition program for Women, Infants, and Children. WIC provides the following at no cost: healthy foods, nutrition education and counseling, breastfeeding support, and referrals for health care and community services.
