Community Outreach Requests

As part of our mission, we promote health by attending various outreach events throughout Lake County. We have program staff available for various meetings, presentation and community events to promote specific aspects of health and disease prevention. Outreach request are based on staff availability. In order to match your request with the appropriate department we need to get more information i.e. location, day time, number of students etc. Please fill out our the outreach form with as much information as possible in case you are still in the planning phase.
