Florida Department of Health in Lake County Administration Office Temporarily Closed

Eustis, FL – The Florida Department of Health in Lake County (DOH-Lake) Administration Building, which houses the Office of Vital Statistics, located at 16140 US Hwy 441, Eustis, FL,    will temporarily close starting Monday, October 24, 2022, until further notice due to construction.

During this temporary closure, the Vital Statistics Office will relocate to the Umatilla location at  249 Collins Street, Umatilla, FL. The regularly scheduled hours are Monday through Friday, 8:00 A.M to 4:30 P.M. If you have any questions or need to reach the Vital Statistics Office, please get  in touch with us at (352) 589-6424. This closure effects no other DOH-Lake offices.
