THE FLORIDA DEPARTMENT OF HEALTH IN LAKE COUNTY TOBACCO PREVENTION AND SCHOOL HEALTH PROGRAMS HAVE MOVED 

Tavares – The Florida Department of Health in Lake County (DOH-Lake) Tobacco Prevention and School Health Programs have relocated from the “Eustis Health Center”, located at 14 North Eustis Street in Eustis, to the DOH-Lake administration building located at 16140 US Hwy 441, Eustis, FL 32726.  The new contact number for these two programs is (352) 589-6424 option number 3 for Tobacco Prevention and extension 2243 for School Health.
