Offering After Hours Back to School Immunization Clinics

EUSTIS, FL To help parents and guardians prepare students to go back to school, the Florida Department of Health in Lake County (DOH-Lake) will be extending their immunization clinic hours on July 28, 2016 till 7 pm at the following locations:
 
July 28th from 1 pm to 7 pm
Leesburg Health Center
2113 Griffin Rd. Leesburg

Umatilla Health Center

Vital Statistics will also be on site at this location for birth certificate services.
 
If not able to attend, we want to remind parents and guardians that immunizations services are offered Monday – Friday at the following DOH-Lake Health Centers: 
 
Clermont Health Center                               
Thursday by appointment only

There is no charge for immunization for children, birth to 18 years of age provided they are still enrolled in elementary, middle or high school.  Parents or guardian must bring a copy of the child’s immunization record. In addition, we offer back to school physical examinations by appointments only.
