2017 CHIP Team Meeting

Join the Community Health Improvement Plan (CHIP) Team in Lake County 

Eustis, FL The Florida Department of Health in Lake County is inviting the public to be a part of health actions that help create and support a culture of health in the community.  The community health teams are seeking people who live, work, or play in Lake County to represent local organizations and neighborhoods by sharing activities related to the top identified community health focus areas:
  • Obesity
  • Substance Abuse
  • Access to Food and Nutrition
  • Access to Primary Care
  • Diabetes

 
Opportunities to participate include upcoming community health improvement plan (CHIP) team focus group events and quarterly meetings at the Lake County Extension Services Building Auditorium, 1951 Woodlea Road, Tavares, FL  32778.

Thursday, April 27, 2017 from 10:00am-12:00pm
Thursday, July 27, 2017 from 1:00pm-3:00pm
Thursday, October 26, 2017 from 1:00pm-3:00pm
